Comprehensive registered nurse home care services for DVA patients. All services are clinically led, individually tailored, and fully funded through DVA for eligible veterans.
Wound Care & Management
Our registered nurses provide expert wound assessment, cleansing, dressing changes, and ongoing monitoring. We manage surgical wounds, pressure injuries, chronic ulcers, and skin tears — working closely with your GP or specialist to ensure optimal healing outcomes.
Medication Management
We assist with medication administration, dose checking, adherence monitoring, and observing for side effects or interactions. This service is especially valuable for veterans managing complex medication regimens or transitioning home after hospital discharge.
Chronic Disease Management
Ongoing nursing review and symptom monitoring for long-term conditions including heart disease, respiratory illness, and multi-morbidity. We coordinate with your GP and specialists, provide health education, and help manage symptoms to maintain quality of life.
Post-Operative Care
Recovery support following surgery — including wound review, pain and recovery observation, mobility assistance, and monitoring for complications. We liaise with your treating team to ensure a smooth, safe transition from hospital to home.
Health Assessments & Care Planning
Comprehensive in-home nursing assessments to evaluate your health needs, develop an individualised care plan, and conduct regular reviews. We monitor changes in your condition and adjust your care accordingly.
Continence Management
Discreet, dignified continence assessment and support, including toileting assistance, skin care, bladder and bowel monitoring, and advice on routines and products. We work to maintain comfort and independence.
Catheter & Stoma Care
Expert catheter maintenance, bag changes, hygiene support, and troubleshooting. For clients with ostomies, we provide stoma observation, pouch changes, peristomal skin care, and output monitoring — delivered by nurses with specialist competency.
Diabetes Management
Blood glucose monitoring, insulin administration support, medication education, hypoglycaemia awareness, and foot-risk observation. We help veterans manage their diabetes safely and confidently at home.
Mobility & Falls Prevention
Nursing support for safe transfers, repositioning, walking assistance during visits, and post-fall assessment. We identify falls risks and work with you to improve safety and confidence in your home environment.
General Clinical Support
Additional clinical services as required — including vital signs monitoring, venipuncture, bowel care, oxygen support, and general clinical observation. Our nurses are equipped to manage a wide range of healthcare needs.
